Systematic review of the intravenous use of preoperative desmopressin in patients at risk of bleeding

ABSTRACT
Introduction: 310 million major surgeries are performed annually and 1 to 4% will have life-threatening complications, with bleeding being one of the main complications; because of this, strategies are sought to reduce post-surgical mortality from this cause, such as the use of desmopressin 30 minutes before surgery. However, this measure is still rarely used.
Material and methods: systematized search in PubMed with key words desmopressin, bleeding, surgery, decreased bleeding, limited to humans, review and experimental articles; obtaining 115 results, selecting 21 and adding two independent search articles on surgery and hemorrhage statistics worldwide.
Results: desmopressin used pre-surgery has been shown to reduce bleeding in cardiac, oncological, otolaryngology, and renal biopsy surgery.
Conclusion: desmopressin administered at doses of 0.3-0.5 μg/kg intravenously 30 minutes prior to the start of surgery or renal biopsy has been shown to reduce bleeding without serious adverse effects.
