Pleural effusion as the initial presentation of mixed pattern non-Hodgkin lymphoma associated with T precursor acute lymphoblastic leukemia

ABSTRACT
Background: Lymph node biopsy is essential for the diagnosis of lymphoma and to subclassify the histological lineage; however, overlapping with acute lymphoblastic leukemia is extremely rare and represents a diagnostic challenge.Clinical case: A 33-year-old male patient with non-Hodgkin lymphoma. The patient manifested dyspnea as initial symptom, finding a massive left pleural effusion on the imaging study. The biopsy revealed a B-cell precursor non-Hodgkin lymphoma with mixed follicular pattern and diffuse lymphoblastic subtype. The bone marrow immunophenotype found a population of 45% of blast cells with heterogeneous CD1a-, CD3cyt+, CD2+/heterogeneous, CD3+, CD7++, HLADR+, CD123+ compatible with T-precursor acute lymphoblastic leukemia.
Conclusions: It is interesting how many non-Hodgkin lymphomas can be a diagnostic challenge as they do not cause B symptoms, so biopsy as an immunohistochemical analysis is essential to guide treatment.
