Delta femoral and deltoid muscle thickness by ultrasonography in nutritional assessment in the intensive care unit

ABSTRACT
Introduction: in critically ill patients, muscle loss is greater in the first days of intensive care unit (ICU) stay. The most performed ultrasound assessment technique is the measurement of muscle thickness at the level of the anterior thigh with the patient in the supine position.
Material and methods: an observational, quantitative, prospective and analytical study, in patients admitted to the ICU of the "Lic. Adolfo López Mateos" ISEM Medical Center from August to October 2020.
Results: a sample of 38 patients was obtained in whom muscle thickness measurements were made on admission to the ICU and on days three, five and seven, both at the femoral level where on admission the mean was 3.56 cm and on day seven it was 2.76 cm; the deltoid muscle, the mean at admission was 1.23 mm and at day seven, 0.93 mm. Significant differences were observed between the three determinations of muscle thickness deltoid (p ≤ 0.001), femoral (p ≤ 0.001), as well as for nitrogen balance (p ≤ 0.001).
Conclusion: the determination of the thickness of the deltoid muscle could be useful in the nutritional assessment of critically ill patients, having only a significant association to identify important nitrogen losses in the first three days, without correlation with positive nitrogen balance; nutritional support monitored by nitrogen balance and/or ultrasound have similar behavioral trends.
