Intestinal actinomycosis: unexpected diagnosis in a schoolchild with type 1 diabetes mellitus. Case report

ABSTRACT
Actinomycosis is a rare infectious granulomatous disease of subacute or chronic course, caused by opportunistic filamentous bacteria of the genus Actynomices, of which Actynomices israelii, an anaerobic gram-positive, non-sporulating and non-acid-alcohol resistant bacillus, is the most frequently isolated. It is characterized by generating tumorous masses that deform the affected area and is accompanied by abscesses that drain a purulent exudate containing the infecting forms called "sulfur granules". The treatment combines tumor resection with antibiotic therapy, achieving a satisfactory evolution. We present the clinical case of an 8-year-old female with type 1 diabetes mellitus, with long-standing abdominal pain and a progressively increasing right periumbilical tumor. There are no signs of peritoneal irritation and multiple treatments have shown no improvement. Diagnostic images showed a mass dependent on the terminal ileum. Surgical excision was performed with bloc intestinal resection; the histopathological study reported chronic enteritis and fibrosis with granules of bacteria of the Actynomices spp. group.
