Anterolateral thigh free flap

ABSTRACT
The anterolateral thigh flap was first described in 1984 by Song et al; even though, its main promoter was Fu Chan Wei. It is used for reconstruction of the head and neck and extremities. Even though this was described for the first time in the 1980s, it did not become popular until the end of the 1990s, and today it is one of the most widely used perforator flaps in soft tissue reconstruction worldwide. The blood supply to the flap comes from the perforating arteries of the lateral branch of the lateral circumflex femoral artery; the venous return depends on the two concomitant veins. The sensitivity of this part of the thigh depends on the lateral femoral cutaneous nerve coming from the L2-L3 roots; and, in terms of motor sensitivity, it is not as relevant because this flap does not generally have muscle. We present the case of a 37-year-old man, with no previous history, who was hospitalized in the plastic and reconstructive surgery area due to a high-voltage electrical burn, after undergoing fasciotomy of the left upper limb indicated for compartment syndrome. There was a bloody area on the left hand and forearm in flexor zone II, III, TIII, IV and V, as well as in extensor zone VIII, with irregular edges. It was decided to reconstruct it using an anterolateral free thigh flap, with good results.
