White stone: superficial mycosis in a pediatric patient

ABSTRACT
Background: White stone is a chronic superficial mycosis caused by yeast fungi
of the genus
Trichosporon. It is a rare, asymptomatic, chronic infection that affects the
hair externally, without invading it, in the form of adherent, soft, whitish nodules. It is
mainly located on the head but can affect other areas less frequently.
Clinical case: 5-year-old female, presenting dermatosis of 6 months of evolution,
located on the scalp, characterized by asymptomatic nodules of 2 to 3 mm, firm, whitish,
adherent, surrounding the hair shaft, at irregular intervals; being treated on multiple
occasions as treatment-resistant pediculosis. Studies were performed on the affected
hair, including direct examination with KOH and fungal culture. The diagnosis of white
stone by
Trichosporon cutaneum was confirmed. Topical treatment was provided, with
excellent results after 3 months.
Conclusions: There are pathologies that produce nodular lesions in the hair, so
white stone should be considered within the differential diagnoses, always making an
integral and timely approach, to avoid a delay in diagnosis, inadequate treatments,
and unnecessary costs.
