Odontogenic keratocyst mimicking lateral periodontal cyst. Report of a case

ABSTRACT
The odontogenic keratocyst is a developmental cyst, it presents aggressive clinical growth and a high recurrence pattern between 25 to 60%. The etiology remains under discussion, being the proliferation of the epithelial dental lamina of the jaws, origin of the enamel organ due to degeneration of the stellate reticulum, before the beginning of the apposition of the enamel and the basal cell component of the oral epithelium. Diagnosis is by radiographic findings. To reach a definitive diagnosis, a detailed clinical history, physical evaluation, clinical photographs, simple X-rays, 3D imaging and histopathology are necessary. It presents as a unilocular or multilocular radiolucent lesion, sometimes expansive with well-defined margins and solitary. The odontogenic keratocyst can present a high recurrence rate of 20%, 30% to 60%. Each of these aspects alone are not enough to reach an accurate diagnosis, which is why it is vitally important not to underestimate the imaging findings and their correlation with histopathology, allowing an early diagnosis of lesions with a high potential for aggressiveness, thus reducing their morbidity. The treatment is controversial, since the literature reports multiple options, both conservative and radical, and adjuvant therapies. A case is presented of a 59-year-old female patient who presented a radiographic finding in the jaw between teeth 3.4 and 3.5, who underwent an excisional biopsy that yielded a histopathological result of odontogenic keratocyst.
