Systemic lupus erythematosus as an initial manifestation in patients admitted to the medical emergency service

ABSTRACT
Background: Systemic lupus erythematosus is a systemic inflammatory disease
characterized by multiple autoantibodies, some of the pathogenic as anti-Ro/La (neonatal
blockade), antiphospholipid (thrombosis) and anti-ADN (nephritis).
Clínical Cases : This study includes a series of cases from our systemic lupus
erythematosus cohort that presented to the emergency room without a previously
established diagnosis. Seven cases were identified as the initial manifestation of the
disease, 5 women and 2 men with a median age of 17 years. For the diagnosis, the
new 2019 ACR/EULAR classification criteria were used, which include seven clinical
manifestations (skin, arthritis, fever, neurological, serositis, hematological and kidney)
and three immunological criteria. Systemic lupus erythematosus is classified with a
score of 10 or more. The manifestations were serositis, arterial thrombosis with associated
antiphospholipid antibodies, psychosis, transverse myelitis, and probable cerebral
vasculitis. All patients received immunosuppressants with high doses of steroids with
subsequent reduction.
Conclusions: Several forms of initial manifestation of systemic lupus erythematosus
have been described. A high index of suspicion for autoimmune disease is necessary
in young subjects, particularly systemic lupus erythematosus.
