Echocardiographic evaluation of septic cardiomyopathy. Importance of biomarkers

ABSTRACT
Introduction. Sepsis-induced myocardial dysfunction (DMS) is one of the most clinically relevant manifestations in sepsis and one of the most early organic dysfunctions in septic shock. Echocardiography allows qualitative and quantitative evaluation of morphology and cardiac function, it is the first line of non-invasive technique for hemodynamic evaluation in septic patients with cardiovascular alterations. Troponin and ProBNP are indicators of severity of myocardial dysfunction.
Methods. A descriptive, observational, cross-sectional study was conducted in patients diagnosed with sepsis and septic shock in the period from May 2018 - May 2019 at the Center for Surgical Medical Research, assessing ventricular function by echocardiography, measuring the values of ultrasensitive troponin I and ProBNP. Out of a total of 54 patients; 32 patients met the inclusion and exclusion criteria.
Results. DMS was present in 14 patients (43.75%) of the sample studied, the average age of the samples was 67±16.5 years, 71.3% had some degree of systolic dysfunction and 100% had alterations in diastolic function of the VI, this group of patients was characterized by higher troponin I and ProBNP values and higher score on the Sequential Organ Failure Assessment (SOFA) scale. Mortality was 50% in these patients (p-0.04).
Conclusion. DMS is common, is related to high biomarker values and high mortality in relation to patients who did not.
