Listeria monocytogenes meningitis in an eldery man

ABSTRACT
Introduction. The acute bacterial meningitis (ABM) is a prevalent disease worldwide, it is always a medical emergency
associated with high morbidity and mortality.
Listeria monocytogenes is recognized in Europe as the third
cause of ABM in adults.
Case presentation. A 73-year-old patient, with a travel history, that started three days prior to poor general condition,
diarrhea, diffuse abdominal pain, fever and 10/10 holocranean headache with sudden spoilage of awareness.
Magnetic resonance imaging of the cranium was performed which reported meningeal reinforcement with the
administration of gadolinium. Patient entered the intensive care unit because he required mechanical ventilation
support. He received empirical treatment with steroids, ceftriaxone, vancomycin and ampicillin.
Lumbar puncture
was performed and cerebrospinal fluid analysis suggested a pyogenic infection. Cerebrospinal fluid (CSF) culture and
multiple bacterial PCR showed
Listeria monocytogenes, the patient continued antimicrobial treatment with ampicillin.
The patient was discharged 21 days later, without neurological sequels.
Conclusions. in patients with risk factors and a suggestive clinical picture of bacterial meningitis due to Listeria,
antimicrobial coverage with ampicillin should be started in order to reduce mortality and complications associated
with this infection.
