Risk of persistence post-treatment of cervical intraepithelial neoplasia with HPV infection 16

ABSTRACT
Introduction: The oncogenesis process for the development of high-grade cervical lesions requires persistent infection of a high-risk human papillomavirus, among which HPV16 is the subtype with the lowest clearing rate.
Objective: To determine if HPV16 infection prior to conization is a risk factor for the persistence of the precursor lesion after treatment.
Methods: In February 2020, a systematic review was carried out on the persistence of disease after LEEP/LLETZ in patients with HPV16 infection prior to treatment. A search was performed in the pubic-medline databases of the National Center of Biotechnology Information (NCBI) and ClinicalTrial.gov of the National Library of Medicine, both in the US. 418 articles were obtained, of which 8 were eligible. All were retrospective cohort. Four articles discussed significant relationship between the presence of infection with HPV16 in the controls prior to conization, and the risk of treatment failure; the remaining four did not talk over this relationship.
Conclusions: There is no agreement in the literature on whether HPV16 infection prior to treatment is an independent factor of lesion persistence. More studies are required, with larger population, to determine if positive HPV16 prior to treatment serves to identify a group at risk of treatment failure.
