Scrofuloderma: A difficult diagnosis case report and literature review

ABSTRACT
Tuberculosis is an infectious disease caused by
Mycobacterium
tuberculosis. Cutaneous tuberculosis is a rare clinical presentation
among cases of extrapulmonary tuberculosis. Clinical subtypes
of cutaneous tuberculosis and its various manifestations are diverse.
The subtypes present diagnostic difficulties. The current
report contributes valuable information as it provides tools for
health professionals to suspect and diagnose cutaneous tuberculosis.
We present a 67-year-old male with a history of pulmonary
tuberculosis. He consulted for xeroderma; however, in the
clinical exploration, we found a fistulized lesion with inverted
borders located on a previous surgical scar. The diagnosis was
achieved by skin biopsy with Ziehl-Neelsen stain. The first phase
of treatment consisted of two months with isoniazid, rifampicin,
pyrazinamide, and ethambutol, and the second phase of
four months with rifampicin and isoniazid. The suspicion of cutaneous
tuberculosis was confirmed as scrofuloderma.
