Angiotensin II in vasodilatory shock: the fourth musketeer

ABSTRACT
Hypotension is a key feature of shock. To date, no vasopressor has consistently been
proven to be superior to the others in terms of clinical outcomes. Angiotensin II (ATII),
an octapeptide, exerts its effects by binding to specific angiotensin receptors. In
humans the major physiological effects are related to maintenance of hemodynamic
stability and fluid and electrolyte regulation. In critical illness, especially in sepsis and
other entities like COVID-19, physiological regulation of renin-angiotensin-aldosteron
system and AT-II is disturbed. Recent studies demonstrated that AT-II is an effective and
safe treatment for refractory hypotension in patients with vasodilatory shock, and may
also have a role in cardiogenic and distributive shock and COVID-19.
