Bleeding and need for transfusion in adults after total knee arthroplasty by subvastus approach who received tranexamic acid

ABSTRACT
Introduction: total knee arthroplasty (TKA) can cause significant bleeding. Tranexamic acid (TXA) is used to decrease bleeding in various settings. However, there is little information about its use in knee surgery.
Objective: to describe the amount of bleeding and the need for blood transfusions in postoperative subjects of TKA by subvastus approach who received ATX.
Material and methods: prospective, observational study. Eighty-two adults undergoing TKA by subvastus approach were included. Data were collected such as: initial BH and 24 hours after the procedure, need for transfusion, number of transfused globular packages and surgical time. A single dose of TXA IV was administered to all patients.
Results: 82 patients who received ATX were included. The median bleeding was 250 mL. Median preoperative hemoglobin (Hb) was 14.4 g/dL, and postoperative was 12.1 g/dL (delta Hb = -2 g/dL). 3.66% required at least one globular package.
Conclusions: this study described the amount of bleeding and the need for transfusion after subvastus TKA using TXA. A historical comparison suggests that TXA decreases postoperative bleeding and perhaps the need for blood transfusion in some patients.
