Surgical treatment vs treatment with yttrium 90 in patients with bilateral gonarthrosis. A self-controlled study. Preliminary report

ABSTRACT

The pathology to articulate, including rheumatoid arthritis and in general the inflammatory diseases, are characterized being sometimes progressive, destructive and that can take to the disability. When in spite of to have instituted a suitable treatment the clinical activity persists, it is possible to be resorted to the ionic sinoviolisis, that consists of the positioning within the affected joint of a substance destined to suppress the sinovial inflammation, by a lapse that oscillates between one and five years. The present work tries to establish the clinical results with yttrium 90 intra to articulate in patients with gonarthrosis, compared with the arthroscopic surgical treatment of the contralateral knee, both, according to scale IKDC. A study is made comparative, longitudinal, prospective, experimental, finite and controlled, carried out in the General Hospital Xoco, patients both sexes, between the 35 and 55 years of age, with complete radiological clinical file, deal with 90 yttrium intra to articulate and previous arthroscopy in contralateral knee, with gonarthrosis degree II, III; using in both scale IKDC evaluated before the procedures and to the 3, 6 and 12 months later to such in the period of July of the 2002 to the month of August of the 2003. Eight complete files were reviewed clinical-radiological, seven women and a man (87.5% and 12.5% respectively), with gonarthrosis degree II seven and one with degree III (87.5% and 12.5% respectively), five rights and three lefts dealt with yttrium 90 (62.5% and 37.5% respectively) and three rights and five lefts with arthroscopic treatment (37.5% and 62.5% respectively). The isotopic sinoviolisis is a procedure simple to make with much smaller morbidity than the surgical synovectomy and that compared with this one gives an equivalent or greater therapeutic success.
