First dorsal metacarpal artery flap for thumb reconstruction

ABSTRACT

Because of its grasping function, the first finger of the hand plays a fundamental role in daily activities. Due to its versatility and constant anatomy, the first dorsal metacarpal artery flap is one of the main reconstructive options for skin defects with bone and tendon exposure, both for the dorsum of the hand, first interdigital space and mainly of the first finger. We present the case of a 27-yearold male patient with a bleeding area on the dorsal surface of the distal phalanx of the first finger of the left hand due to a saw injury, with bone exposure and loss of 90% of the nail bed, as well as disinsertion of the extensor pollicis longus tendon. Successful reconstruction was performed with a flap of the first dorsal metacarpal artery and reinsertion of the extensor pollicis longus tendon with the pull-out technique. We conclude that the first dorsal metacarpal artery flap is a versatile and safe reconstructive tool that allows restoration of coverage with sensibility in cutaneous defects of the hand.
