Installation of vein central catheter as quality standard in nursing care

ABSTRACT
At present, infirmary staff is seeking risk decrease during patients’ attention in order to improve care quality. This implies to establish rules to develop the performed procedures, which sometimes mean an added risks for the patients. Therefore, setting up quality indicators in infirmary care has been one of the main goals of our current profession. This way, the installation of a central venous catheter has been catalogued as a basic process being developed by the infirmary personnel, so it is very important that the staff performs it following quality standards. Methodology: A descriptive, observational, longitudinal and prospective study was carried out in two stages; the first one went from November to May 2001 and it was performed through the direct observation with the application of an assessment form designed for such a purpose and validated by a leader proof where an Alpha Combrack of 0.41 was obtained. The universe was constituted by the infirmary staff performing the central venous catheter installing in different hospitalization services. According to the obtained results, a tactic plan is structured. It includes personnel training in relation to the process, which was developed through classes and booklets. A second observational stage was performed following the same criteria. Results: In the first stage, the global efficiency index was of 61%, in the second step the quoted index was of 87.6%, what means that the procedure was carried out with a greater efficiency. The effectiveness index per activity when installing the central catheter was of 55.7%, increasing up to 89.8% after the instructional training. The obtained increasing was of 32.8%. Conclusions: When establishing a performing standard in installing a central venous catheter, an increasing in quality infirmary care was observed; in addition to this, periodic evaluation and the implementation of steady improvement strategies allow us to keep or increase the standard.REFERENCES
