Use of gelatin sponge ''Gelfoam'' as a scaffold in combination with autograft for the treatment of pseudoarthrosis, case report

ABSTRACT
Introduction: the current management of pseudarthrosis has become a challenge for Orthopedists, since there is no treatment that can guarantee bone consolidation, being necessary in these cases the use of orthobiologics, which together with optimal biomechanical conditions are a fundamental part of the treatment of pseudarthrosis.
Case report: in this report we present two cases, both male, 25 and 23 years old, with diaphyseal fracture of the femur following a motor vehicle accident, the patients underwent surgery, but with evolution towards pseudarthrosis. Both patients were treated by removal of osteosynthesis material, debridement of non-viable bone tissue and subsequent centromedullary nailing, with placement of iliac crest autograft plus bone marrow aspirate, using a gelatin sponge as graft container and scaffold, with which both obtained a faster total bone regeneration and consolidation.
Conclusions: we highlight the use of the gelatin sponge as a containment membrane and ideal medium for the transport of growth factors and osteoblast cell differentiation from the autograft, which could act in synergy accelerating the process of bone consolidation, improving the success rate in the treatment of this pathology.
