Case report of management of the traumatic foot bilateral transmetatarsal amputation in a Second Level Hospital

ABSTRACT
A complication of transmetatarsal amputation is the stump deformity in equine, which makes plantigrade gait difficult. Achilles – plantar complex contracture is the cause. The Silverskiold test is important for surgical planning. The Barouk technique is effective in gastrocnemius contracture in 95%. Achilles zetaplasty carries greater risks. A male patient is presented with crushing of the forefoot with a metal piece of 1 and a half tons, with bilateral traumatic foot by symptoms, necrosis of the toes is observed. A bilateral transmetatarsal amputation was performed, 3 months later a right percutaneous zetaplasty was performed and the medial gastrocnemius was released with the Barouk technique with the placement of a circular cast in the last Ponseti position. Currently, the patient is shown with a plantigrade gait, without equine deformity, with full support, without use of third support, without pain on plantar flexion, dorsiflexion of 0°, surgical wounds in a complete state of healing, with the use of an orthosis. We conclude that it is necessary to perform tenotomies in patients with transmetatarsal amputation to avoid the equine and improve plantigrade gait.
