ABSTRACT
The one anastomosis gastric bypass or mini-gastric bypass is a bariatric procedure that has gained popularity in recent years, with efficacy and safety comparable to Roux-en-Y gastric bypass. Petersen's hernia after mini-gastric bypass is a rare complication, so there is not consensus on its routine closure. There are few reported cases, most of them being an incidental intraoperative finding after a picture of chronic abdominal pain, persistent biliary gastroesophageal reflux, or intestinal occlusion. We present the case of a 58-year-old female with a history of mini-gastric bypass 10 years previously with intestinal occlusion and a Petersen hernia with segmental intestinal ischemia, meriting resection and intestinal anastomosis and a conversion to Roux-en-Y gastric bypass with closure of mesenteric defects.
