Ventricular circulatory assistance. Experience of the HSMU Cardiology

ABSTRACT
Introduction: the ventricular assist device has proven to be the only effective therapeutic resource for the management of severe ventricular and/or biventricular failure that does not respond to maximum pharmacological treatment and aortic counterpulsation.
Objective: to report the experience of the placement of ventricular assist devices in patients with heart failure refractory to usual treatment of the High Specialty Medical Unit (HSMU) of the Hospital de Cardiología del Centro Médico Nacional Siglo XXI.
Material and methods: in accordance with the program established by the HSMU cardiology for ventricular assist device placement, a retrospective case series of 52 adult patients, of any age, who required a ventricular assist device was carried out; the type of device, indications, complications and time of use of the ventricular assist device were evaluated.
Results: 4 types of device were placed: Levitronix (15%), ECMO (75%), Vitacor (4%) and HeartMate II (6%). 100% were in class 1 of the INTERMACS classification; 78% were bridge to recovery, 14% bridge to transplant and 6% definitive therapy. Survival was 48% and complications were: renal failure, infections, ventricular assist device dysfunction, and bleeding.
Conclusions: ventricular assist devices are a first-world tool with satisfactory short- and long-term results.
