Characterization of elderly patients with post-surgical acute confusional syndrome

ABSTRACT
Introduction: The incidence of acute confusional syndrome in the elderly rangesfrom 10% to 40%, depending on whether they are outpatients or inpatients, and according to services and types of treatment.Objectives: To characterize elderly patients with post-surgical acute confusionalsyndrome.
Methodology: A prospective descriptive study of a series of cases was carried out in 96 elders with post-surgical acute confusional syndrome in the General Surgicalwardfrom Vladimir Ilich Lenin Teaching General Hospital, out ofa universe of 360 elderly patients who were operated on from January 2015 to January 2019.
Results: The most affected age group was 75-89 years, being the male sex the most predominant, which represented 53.1%. The most comon underlying condition was arterial hypertension (59.3%). The most frequent symptom was attention deficit in 61.4% and the evolution time was 24-48 hours in 42.7%. Older adults who underwent major emergency surgery (59.3%), with a surgical time longer than 2 hours (45.8%), had a higher incidence of acute post-surgical confusional syndrome. The hospital stay lasted one week or more in 43.7% and 86.5% of the patients received therapeutic treatment.
Conclusions: Acute confusional syndrome is a frequent complication in the elderly people and its incidence dependson the type of surgery. The condition can also appear as the result of an adverse outcomeduring the immediate and long-term post-operative period.
