Disseminate gonorrhea: a clinical case and subject revision

ABSTRACT
A 23 year old man was admitted to the hospital because of dysuria, purulent urethral discharge, fever, polyarthralgias, tenosynovitis and dermatitis. The skin lesions were less than a dozen, distributed on the distal portion of extremities and around the joints. They turned into pustules on an erythematous haemorrhagic base.
Neisseria gonorrhoeae was cultured from the urethral exudate as well as from the endocervical exudate of the patient´s new sexual partner. Haemolytic complement deficiency may predispose individuals to disseminated gonococcal infection or recurrent neisserial sepsis. A ten days course of ceftriaxone, with proven activity against penicillin – resistain gonococci, was applied to this patient with good results. Diagnosis of gonococcal urethritis depends on microscopic examination of gram – stained smears, which may show characteristic gram – negative diplococci in the cytoplasm of neutrophils but culture – isolation of
N. gonorrhoeae is the gold standard. Epidemiologic research has confirmed the high prevalence of
N. gonorrhoeae infections in the female sexworkers 13.6% and the homo – or bisexual men 3.3%. Health personnel must be made aware of the most effective case finding techniques and the rapid search for sex contacts of patients, and to educate the public about venereal diseases.
