Corruption and health 4: leading the integral Health Care System in Guatemala: the good, the bad and the ugly

ABSTRACT
Introduction: The traditional advice for outstanding professionals who wish to transform health systems and improve population health is to disregard government jobs to avoid reputation damages, professional stagnation, accusations or involvement in corruption, threats or attacks for changing the status quo, ending in prison being innocent or placing their lives at risk.
Objectives: analyze the work experience in an executive position within the Ministry of Health and Social Welfare in order to report achievements, challenges and lessons learned, and develop recommendations to equip others to enhance health system performance.
Development: the Author’s experience leading the Integrated Health Care System during six months is analyzed. Initiatives that change the status quo and improve the productivity and efficacy of services are described. Barriers and obstacles that impede enhancements in system efficiency and effectiveness are identified. A matrix is built with “good, bad and ugly” qualities to describe the challenges to practice effective executive health leadership and resolve inefficiencies in governmental administration and corruption in public management.
Conclusions: The public health system in Guatemala fails due to lack of vision of changes needed, incompetence in officials hired by influence instead of track record, corruption networks embedded at all levels, lack of ethics and values, and indolence and disregard from officials in power towards the suffering, ailments and needs of the population. Experts known as “unfiltered leaders” are needed to change the situation and transform the system.
