Sthesiometric neurodynamic method at assessing cirrhotic patients

ABSTRACT
Objective: To describe the use of the Sthesiometric Neurodynamic Method (SNM) in patients suffering from cirrhosis.
Methods: A descriptive, prospective, cross-sectional study was carried out with cirrhotic patients treated at Centro de Investigaciones Médico Quirúrgenas, in a period of three years (2014 - 2017). A total of 45 subjects were analyzed, 30 of them cirrhotic and 15 seemingly healthy subjects. Of the cirrhotic subjects, 23.5% had history of at least one episode of hepatic encephalopathy. The variables studied were age, sex, level of education, etiology and stage of cirrhosis, history and number of episodes of hepatic encephalopathy, and sthesiometric variables. The measures of central tendency, dispersion and frequency distributions were estimated for the statistical analysis. Student's t test was used to compare variables related to cerebral cortical neurodynamics, Fisher's exact test was used to determine the significance of cerebral cortical neurodynamics, and the values of the length of the plateau of the curve were compared for the analysis of variance, according to the stage of cirrhosis.
Results: The study showed that the plateaus of cirrhotic subjects were lower than those of apparently healthy subjects, who had typical plateaus. 17% of cirrhotic patients exhibited atypical plateaus and flat curves, which showed impairment of cerebral cortical neurodynamics in this group of patients. Cirrhotic cases with history of hepatic encephalopathy, males, and those older than 40 years had more cortical neurodynamic conditions. Healthy patients, with intermediate or university level of education, showed higher values of plateau length than those of middle and middle-high level.
Conclusions: The most frequent causes of liver cirrhosis were hepatitis C virus and alcoholism. Most of the cirrhotic patients had cerebral cortical neurodynamic involvement. There was no evidence of relationship between the stage of liver cirrhosis and the level of cerebral cortical neurodynamic involvement. There was no impairment of cerebral cortical neurodynamics in apparently healthy patients.
