Comparative study between the modified Dakin’s solution and povidone-ione for treatment 97 of infected wounds in patients subjected to appendicectomy

ABSTRACT
Objective: To compare the efficacy of the modified Dakin’s solution with that of povidone-iodine (PVP-I) in the treatment of infections at the incisional site in patients subjected to appendicectomy.
Setting: Third level health care general hospital.
Design: Prospective, comparative, randomized study.
Statistical analysis: Chi square, Fisher’s exact test, and Student’s t test were used.
Patients and methods: Patients were randomly assigned to two groups: Group I patients treated with Dakin’s solution; Group II patients treated with povidone-iodine (PVP-I). We assessed time of treatment, number of colony forming units per gram of tissue (CFU/g), cultured bacteria, pain during cleansing of the wound, and secondary reactions.
Results: Groups were similar in the distribution by age, gender, and number of (CFU/g), at the start of treatment. Group I patients required less treatment days (p = 0.003). Ten days of treatment were enough in 84% of Group I patients to suture the wound, as compared to only 23% of patients from Group II (p = 0.005). The second (cfu/g), assessment revealed significant less values in group I patients (p = 0.05). Patients of group I reported less intense pain starting on the 6th day of treatment (p = 0.005). No collateral effects were observed.
Conclusion: The modified Dakin’s solution reduces treatment time for infections in the superficial incisional site, restores bacterial equilibrium in less time, and provides better tolerance to washing procedures.
