Usefulness of ultrasound versus biopsy in the diagnosis to sentinel node metastasis in patients with breast cancer

ABSTRACT
Introduction. Sentinel lymph node ultrasound can be useful in the early
detection of breast metastases.
Objective. To compare the usefulness of ultrasound versus sentinel node
biopsy in the diagnosis of sentinel node metastases in patients with breast
cancer.
Materials and Methods. A retrospective, observational, descriptive
and cross-sectional study was carried out in a group of 148 patients with
breast cancer registered in the Venezuelan Anticancer Society January
2018-December 2020.
Results. It was found that the largest number of cases with lymph node
metastasis in those older than 40 years (83.1%). Infiltrating ductal carcinoma
was the most frequently found entity (83.8% [n = 119]) (
p=0.03). The
diagnostic concordance of the preoperative ultrasound for the correct
diagnosis of a sentinel node was 0.28, which according to the Kappa scale is
acceptable (
p=0.001). Taking biopsy as gold standard, the correct diagnosis
of the sentinel node by ultrasound was 66.4%, 95% confidence interval (95%
CI) from 57.7% to 74.2%, the sensitivity was of 47.3% (95% CI, 33.9%
-61.1%), the specificity of 79.7% (95% CI, 68.9% -87.5%), the positive
predictive value was 61. 9% (95% CI, 45.6% -76.0%) and the negative
predictive value was 68.5% (95% CI, 57.8% -77.5%).
Conclusion. Ultrasound has an acceptable utility in the diagnosis of sentinel
node metastasis.
