Septic arthritis in pediatric patient

ABSTRACT
Introduction. Septic arthritis is an inflammatory reaction of the joint
space caused by colonization of the joint cavity by a microorganism. The
most common cause is hematogenous spread.
Case report: a 3-month-old
male patient who comes to the emergency department of Playa del Carmen
General Hospital, Q. Roo for presenting pain and functional disability of
the lower left limb, sudden crying, and fever, without an infectious focus.
On physical examination, there was an increase in the volume of the hip
joint, limitation of the arc of joint movement, flexion, and fever. Palpation
with increased local joint temperature. In the laboratories, leukocytosis
of 24,300 × 109/L was found, semi-quantitative PCR of 48 mg/L, data
of an infectious process. In the emergency ultrasound, joint effusion was
detected in the hip. A diagnosis of septic arthritis of the left hip was made
and surgical treatment and antibiotics were indicated.
Discussion. Septic
hip arthritis is an infrequent pathology and its timely diagnosis, as well
as the indicated emergency treatment, is the key to the favorable clinical
evolution of the patient, the standard treatment is arthrotomy of joint
drainage as well as joint washing, empirical antibiotic management, and
later oriented to the isolated agent. In this case, the arthrotomy pathway
was selected by draining and washing the joint as soon as possible. The
arthroscopic route offers greater advantages as it is a minimal access
surgery, which meets the objectives of the surgical treatment and preserves
the anatomy of the joint.
