Epidemiology and prognostic factors in patients with sepsis

ABSTRACT
Introduction: Sepsis is a life-threatening organic dysfunction, caused by a deregulated host response to infection.
Objective: To identify the prognostic factors of the patients suffering from sepsis.
Methods: A descriptive, prospective and cross-sectional study was carried out with patients treated at the Progressive Care Service of Juan Bruno Zayas Alfonso General Surgical Hospital, in Santiago de Cuba, who suffered from sepsis. The research was carried out from April 2017 to April 2019. The following variables were analyzed age, sex, state on discharge, case fatality rate, use of mechanical ventilation, SOFA and qSOFA scores. The data processing was done automatically using the SPSS version 10.0 package. Data univariate analysis included calculation of mean and standard deviation for numerical variables and for categorical rates and proportions. The chi-square statistic calculation and the identification of factors associated with sepsis were used for divariate analysis; likewise, p value and 95% RR for confidence interval were calculated.
Results: 41.7% of cases died. Male patients predominated (52.8%) and the average age was 68 ± 12 years. There was a statistical association between the qSOFA score at admission, with the status at discharge of the patients. This score was positive in 53 patients (73.65%), 26 of them died. SOFA score related the most to mortality since it increased as the said score increased. There was no statistical association between SOFA score, the use of mechanical ventilation and mortality. 51.9% of the patients who did not receive mechanical ventilation and 37.8% of those who did receive it died.
Conclusions: The prognostic factors identified in the patients with sepsis were qSOFA positive and SOFA ≥ 6 points. It was found that when SOFA and qSOFA scores were higher, the risk of mortality of patients with sepsis increased.
