Quality of care in the emergency service of the ''Emilia de Córdova y Rubio'' Polyclinic

ABSTRACT
Introduction: The evaluation of the quality of care is an enduring concern of health systems. Its conceptual and methodological bases are approached from the structure, process and results dimensions.Objective: To assess the quality of medical care in the emergency service at Emilia de Córdova y Rubio Community Clinic.
Methods: A study was carried out on health systems and services in the emergency area at Emilia de Córdova y Rubio Teaching Community Clinic, Mayebeque, from April 1, 2017 to March 31, 2018. The variables used were the structure, processes and results in the emergency department. The research universe consisted of 51 workers. One hundred sixty-four patients were surveyed, they were selected by a simple random probability sample. An observation guide and questionnaires were applied to assess the structure and satisfaction of patients and providers. Internal audit was used to evaluate the processes; and lethality indicators of tracer diseases were used to evaluate the results. Absolute and relative frequencies were used for the statistical analysis.
Results: The structure was evaluated as reasonable (62.3%), with difficulties in cleaning; ventilation; bathroom, ceiling and wall conditions; as well as in order and comfort in the waiting room and the completion of non-medical personnel. All aspects of the care process were positive. Acute myocardial infarction was found to have high lethality (30.76%). The patients’ satisfaction perceived was poor (55.55%), providers satisfaction was classified as average (75%), due to management difficulties, organization, discipline, conditions in the waiting room and care areas.
Conclusions: The quality of care in the emergency department was average. The elements evaluated as deficient, significantly affected the structure variable and the level of satisfaction of users and providers.
