Villonodular synovitis in the shoulder joint. Report of a case

ABSTRACT
Introduction. Pigment hairy-nodule synovitis is a proliferating disorder which
affects joints, tendinous sheaths, and bursas. The most affected joints are: knee,
hip, and fingers. According to literature reviews, shoulder is the least affected
part of the body.
Material and methods. In the present article, we report a
case placed in glenohumeral joint, from a 77 year-old patient, who also presented
rheumatic arthritis. Having clinical symptoms characterized by a profuse joint
extravasation, this patient was subjected to shoulder arthroscopy, through
biopsies and the histophatology diagnosis of diffuse pigment hairy-nodular
synovitis was corroborated.
Discussion. Due to the rare form of shoulder
presentation, it is very difficult to define the clinical findings and
diagnosis support; however, our case, the same as most general cases
reported up to the present, it shares: slow clinical evolution, general
pain presence and no specific ache in shoulder, volume increase, and area
asymmetry. In every case the anatomic-pathological findings show long
fuzziness, extended from the peripheral osteocondral union to the joint,
with a brown coloring, sometimes sanguinolent and whose microscopic
description shows a proliferating synovia, fibroplasts or primitive
mesenchymatous cells, what finally offers the specific diagnosis.
