Hypereosinophilia and increased IgE: atypical manifestations of primary cutaneous peripheral T-cell lymphoma

ABSTRACT
Background: Primary cutaneous peripheral T-cell lymphoma not otherwise specified
is a heterogeneous group that includes all T cell neoplasms that do not fit into
any of the best-defined subtypes in the WHO-EORTC classification. It represents only
about 2-6% of primary cutaneous lymphomas and exhibits an aggressive behavior and
unfavorable prognosis, and its diagnosis is usually delayed several years due to the
non-specific histopathological and immunohistochemical findings, especially in the
initial biopsies, so it represents a diagnostic challenge.
Clinical case: A 36-year-old male patient with primary cutaneous peripheral T-cell
lymphoma not otherwise specified whose diagnosis was masked by the presence of
hypereosinophilia and IgE elevation, confirmed after 7 years.
Conclusions: Due to the rarity of cases, unspecified primary cutaneous peripheral
T-cell lymphoma is a disease that to date is not well understood and, consequently,
algorithms for its diagnosis and treatment are lacking.
