Exercise Stress Test under Electrocardiographic control. Review of International Guidelines and Cuban Standards

ABSTRACT
A review of the main international guidelines and of the Cuban standards referring to the Exercise Stress Test (ExT) under electrocardiographic control was carried out with the objective of updating the corresponding protocol of action in our country. The indications of these tests are reviewed, with greater emphasis on their diagnostic value and therapeutic and prognostic evaluation in coronary heart disease, as well as in arterial hypertension, cardiac arrhythmias, congenital heart disease and in Sports Medicine. The sensitivity and specificity of such tests and their relationship with the number of diseased coronary vessels, as well as the value of the determination of the functional capacity of the subject evaluated by its value in determining the status of cardiac function are indicated: decrease in a 50% or more correspond to left ventricle systolic dysfunction.
The absolute and relative contraindications of ExT, as well as its interruption criteria, that when are well applied reduce their risk of complications. The clinical and electrocardiographic criteria of myocardial ischemia during physical exercise are cited, as well as that additional hemodynamic elements are offered to make a better interpretation of such test.
Finally, a mention is made about Cardio-respiratory ExT or ergospirometry and its value is indicated in the objective evaluation of functional capacity in healthy, sick individuals and athletes, by determining oxygen consumption, as well as in the prognostic stratification of patients with cardiovascular diseases, particularly with heart failure.
It is concluded that the ExT remain fully valid when considering their diagnostic and prognostic utility, as well as their wide employment, low cost and low risk.
