Acute heart failure during COVID-19. Pathophysiology

ABSTRACT
At the end of 2019, a group of patients suffering from pneumonia that by their characteristics resembled viral pneumonia was reported in Wuhan, Hubei, China. The infectious germ was quickly identified as a coronavirus called SARS CoV2 and the disease as COVIG 19. This new virus affects multiple organs, including the heart, causing in some cases acute cardiac damage and sometimes death. Several colleges of cardiology worldwide have spoken in medical consensus to guide diagnostic and therapeutic points of view in this new disease. The objective of this work is to make a review as exhaustive as possible to know the pathophysiology of acute heart failure and cardiogenic shock as its most severe expression and the alterations it causes at the level of all organs, a fundamental element in trying to prevent or treat once it is established. Method. We did a comprehensive review of the literature by reviewing important databases such as Medline, Pubmed, Scielo, Cochrane, National Institute for Health and Care Excellence (NICE) and others. In addition, we reviewed randomized and controlled clinical trials, systematic review of controlled clinical trials, meta-analysis and high-quality cohort study, as well as observational study and systematic review of observational studies, from January 2010 to March 2020. Results: after an analysis to include and exclude research papers, a total of 270 relevant articles were found. Most of the excluded articles did not contain conclusive information regarding the criteria for acute heart failure, cardiogenic shock and its pathophysiology. Conclusions. Knowing the alterations that occur in acute heart failure and cardiogenic shock as its most severe expression, allows us to know the impact it has on the different systems of our body, a fundamental action to try to prevent or treat this syndrome in time, and thereby decrease its high mortality.
