Comparative study between cefepime/metronidazole versus cefotaxime/metronidazole for the treatment of complicated intra-abdominal infections

ABSTRACT
Objective: To establish a comparison between cefepime/metronidazole and cefotaxime/metronidazole in the treatment of complicated intraabdominal infections.
Design: Case series report.
Setting: ICU of a tertiary care hospital, Mexico City.
Patients: Sixty patients with intraabdominal infections (27 men, 33 women, mean age 51.4 ± 17.3 yrs) were studied.
Interventions: The two groups of patients received metronidazole 1 g/kg IV (initial dose) and thereafter 7.5 mg/kg every 6 hours, and randomly received cefepime 2g IV every 12 hours (group 1) and cefotaxime 2 g IV every 8 hours (group 2) during hospital stay.
Results: It was observed APACHE II score 11.2 ± 6.3 vs
11.7 ± 6.4 points (p = 0.764); clinical improvement 19, 61.3% vs
21, 72.4% (p = 0.375); and bacteriological improvement 10, 32.3% vs
10, 34.5% (p = 0.754); no patient had side effects related with the use of antibiotics. Mortality rate was 5, 16.1% vs
6, 20.7% (p = 0.651).
Conclusion: The combination cefepime/metronidazole and cefotaxime/metronidazole had similar efficacy and safety in the treatment of patients with intraabdominal infections.
