Tratamiento del traumatismo craneoencefálico cerrado severo en niños.Estudio de casos y controles de 20 pacientes

RESUMEN
Introducción: El trauma craneal cerrado severo es causante de una tasa alta de morbilidad y letalidad, con estancia hospitalaria prolongada. Los procedimientos terapéuticos han variado en los últimos años. El objetivo del estudio es evaluar la efectividad de dos diferentes procedimientos terapéuticos.
Lugar: Servicio de Urgencias de un hospital pediátrico, Hermosillo, México.
Diseño: Prospectivo.
Material y métodos: Se revisaron 153 expedientes con diagnóstico de traumatismo craneal grave, separando los traumatismos craneal cerrado graves, 10 pacientes para el grupo control y 10 para el grupo de estudio. Se registró la mortalidad, tiempo de corrección de la T/A, días de estancia con ventilación mecánica, días de estancia hospitalaria, alteraciones en los niveles séricos de sodio y glucosa.
Resultados: La mortalidad en ambos grupos fue similar (p = 0.58). El tiempo de corrección de la T/A no mostró diferencias (p = 0.12). En la glucosa en sangre hubo incremento significativo en el grupo de control con respecto al de estudio (p = 0.01). La estancia hospitalaria, el tiempo de asistencia a la ventilación fue superior en el grupo de control (p = 0.001).
Conclusiones: La guía terapéutica empleada en el grupo de estudio mostró mejores resultados.
