Tratamiento no operatorio del trauma hepático. Experiencia inicial con cinco pacientes

RESUMEN
Objetivo: Describir la experiencia inicial con tratamiento no operatorio en cinco pacientes con trauma hepático cerrado.
Diseño: Descripción de casos.
Sede: Hospital de tercer nivel de atención.
Descripción de casos: Se describen los casos clínicos de cinco pacientes que sufrieron trauma abdominal con lesión hepática de diversos grados que llenaron los requisitos establecidos para recibir tratamiento no operatorio. Los cinco pacientes fueron hombres, dos sufrieron trauma cerrado y tres penetrante; a todos se les realizó tomografía computada y en un caso laparoscopía diagnóstica. Se encontraron tres lesiones grado I, una grado II y una grado III, con líquido libre en cantidad menor a 200 ml en cuatro y 500 ml en uno. Los resultados fueron favorables en todos los pacientes.
Conclusión: El tratamiento no operatorio de lesiones hepáticas ha probado ser efectivo si se cumplen todos los requisitos para su aplicación y se cuenta con los medios hospitalarios necesarios para el manejo multidisciplinario de estos pacientes.
