Relación apolipoproteína B/Apolipoproteína AI y componentes del síndrome metabólico en adolescentes con obesidad: Efecto de una dieta hipocalórica baja en carbohidratos

RESUMEN
Objetivo: Determinar los efectos de una dieta hipocalórica baja en carbohidratos sobre la relación apolipoproteína B/Apolipoproteína A-I y componentes del síndrome metabólico en adolescentes con obesidad.
Material y métodos: 10 adolescentes con obesidad, edad promedio 17.9 años. Se determinó la presencia de síndrome metabólico (NCEP-ATP III). Se medió de manera basal y 6 semanas después de una dieta hipocalórica baja en carbohidratos la composición corporal, glucosa, lípidos, ácido úrico, insulina, HOMA, apolipoproteína B, A-I y la relación apoproteína B/apoproteína A-I.
Resultados: 7 de los 10 participantes cumplieron criterios de síndrome metabólico. El peso corporal, IMC, perímetro de cintura y porcentaje de grasa corporal disminuyeron significativamente (
p 0.0001) al igual que glucemia de ayuno (
p 0.02), glucemia 2 horas postcarga (
p 0.0001), triglicéridos (
p 0.04), HDL-colesterol (
p 0.03) insulina (
p 0.04) y HOMA (
p 0.03). La relación apo B/Apo AI disminuyó 7%. En individuos con reducción de peso › 8 kg la apolipoproteína B disminuyó 8.8% y la relación apo B/Apo AI en 10.9%. En sujetos con reducción del IMC › 3 la apolipoproteína AI aumentó 5.4%, apolipoproteína B disminuyó 11% y la relación Apo B/Apo AI disminuyó 16.8%.
Conclusiones: La dieta hipocalórica baja en carbohidratos disminuye en un corto periodo de tiempo la relación Apo-B/Apo-AI, modificando favorablemente los componentes del síndrome metabólico en adolescentes con obesidad.
