Trombosis de vena porta posterior a cirugía laparoscópica y cirugía bariátrica: Revisión de la literatura

RESUMEN
Introducción: La trombosis de vena porta es una complicación rara pero potencialmente letal después de procedimientos laparoscópicos. Pacientes con obesidad sometidos a cirugía bariátrica tienen un mayor riesgo de presentar tromboembolismo venoso debido a un estado hipercoagulable e inflamatorio. Los procedimientos bariátricos no están libres de complicaciones, pues se informa que su incidencia es tan alta como 7%.
Material y métodos: Se realizó una búsqueda electrónica en las plataformas de PubMed, Medline, Central, Embase, Ebscohost y Cochrane Central Database de artículos publicados de enero de 2000 hasta el mes de diciembre de 2017. Se utilizaron las siguientes palabras clave:
Portal vein thrombosis, laparoscopic surgery, gastric bypass in Roux-en-Y, complications treatment, management y diagnosis. Se realizó un análisis de 45 artículos y se llevó a cabo esta revisión haciendo énfasis en la evaluación, diagnóstico y tratamiento de trombosis venosa portal secundaria a cirugía bariátrica laparoscópica.
Conclusiones: El diagnóstico y el tratamiento de trombosis venosa portal secundaria a cirugía laparoscópica para prevenir la isquemia intestinal y posible necrosis no deben demorar. Por su amplio rango de síntomas se requiere un alto índice de sospecha. La terapia anticoagulante generalmente es satisfactoria para prevenir la extensión del trombo, logrando una recanalización parcial y evitando nuevos eventos trombóticos.
