Linfoma de la zona marginal, tres enfermedades bajo un mismo nombre: presentación de dos casos y revisión de la literatura

RESUMEN
El linfoma de la zona marginal es un conjunto de neoplasias con
tres subtipos establecidos: nodal, extranodal y esplénico; cada uno
con características histopatológicas, citogenéticas y clínicas propias.
El día de hoy se considera al linfoma de Hodgkin como la
neoplasia con un mejor pronóstico en los últimos 10 años, llegando
a ser una enfermedad controlable casi en su totalidad. El subtipo
esplénico es el más raro de esta clase de linfomas, siendo ‹ 1% de
todos los linfomas no Hodgkin. Se presenta el caso de una paciente
de 64 años de edad, cuya manifestación más importante era dolor
abdominal, se realizó tomografía axial computarizada simple que
reveló esplenomegalia de 22 cm en su diámetro mayor. Por otro
lado, el subtipo nodal se caracterizó por una localización primaria
en ganglios linfáticos en ausencia de infiltración a otros órganos. Se
presenta el caso de un paciente masculino de 56 años de edad con
manifestaciones de sinusitis crónica, se realizó tomografía axial
computarizada simple de cráneo en donde se identificó un tumor
de 4 x 2 cm localizado en el seno maxilar derecho. El extra nodal
asociado a tejido linfoide asociado a mucosas, es el más común de
los tres y está muy relacionado con ciertas infecciones como Helicobacter
pylori en linfoma primario gástrico y enfermedades inflamatorias
crónicas.
