Colecistitis aguda acalculosa en el paciente quemado

RESUMEN
La colecistitis aguda acalculosa es una complicación no rara en el paciente quemado. Es secundaria a atonía e isquemia de la vesícula biliar que evoluciona a un proceso necroinflamatorio que habitualmente se complica con infección biliar y sepsis. Clínicamente se manifiesta con dolor y plastrón en cuadrante superior derecho del abdomen, fiebre, leucocitosis e incremento de bilirrubina y fosfatasa alcalina. El ultrasonido de vesícula y vías biliares es el estudio de elección para corroborar el diagnóstico. El tratamiento temprano es fundamental para disminuir la morbi-mortalidad de esta entidad y es mediante el empleo de antibióticos, dependiendo de las condiciones del enfermo aunado a: colecistostomía percutánea, drenaje endoscópico o colecistectomía laparoscópica.
