Tratamiento de la hemorragia posparto con condón hidrostático intrauterino

RESUMEN
Objetivo: reportar la experiencia con el condón hidrostático intrauterino
en el control de la hemorragia obstétrica.
Material y Método: estudio descriptivo y retroprolectivo efectuado
del 1 de marzo al 27 de agosto de 2015 en un hospital rural de México.
El condón hidrostático intrauterino se aplicó a pacientes con hemorragia
obstétrica sin respuesta al tratamiento médico.
Resultados: se identificaron 955 pacientes en puerperio posparto
vaginal de las que 40 (4.1%) ameritaron la aplicación del condón hidrostático
intrauterino. El método no tuvo éxito en 2 de 40 pacientes
(5%); una de ellas requirió histerectomía obstétrica de urgencia y la
otra laparotomía exploradora con medidas conservadoras. Se encontró
que 11 de 40 pacientes (27.5%) requirieron, al menos, la transfusión
de 1-3 paquetes globulares. Ninguna de las 38 pacientes (95%) que
reaccionaron favorablemente al método tuvo datos de endometritis en
el seguimiento del puerperio ni complicaciones asociadas con el uso
del condón hidrostático. Durante el periodo de estudio no se registraron
muertes maternas.
Conclusión: el uso del condón hidrostático intrauterino es un método
eficaz para el control de la hemorragia obstétrica posparto vaginal
secundaria a atonía uterina sin respuesta al tratamiento médico.
