Manejo laparoscópico del quiste hepático gigante. Revisión de la literatura

RESUMEN
Introducción: Los quistes simples son vistos en menos del 5% de la población general. Éstos se observan principalmente en pacientes mayores de 40 años, y alcanzan tamaños gigantes casi con exclusividad en los mayores de 50 años. Sólo el 15% de dichos quistes son sintomáticos. Los quistes hepáticos son el principal hallazgo clínico en la enfermedad poliquística del hígado y la manifestación extrarrenal más frecuente de la enfermedad poliquística renal autosómica dominante.
Material y métodos: Se hizo una búsqueda electrónica en las plataformas de PubMed, Medline, Central, Embase, Bandolier y Cochrane Central Database de artículos publicados de enero de 2000 hasta el mes de noviembre de 2016. Se utilizaron las siguientes palabras clave:
giant, hepatic, cyst, liver, treatment, management, diagnosis y laparoscopic. Se realizó un análisis de todos los artículos y se llevó a cabo esta revisión haciendo énfasis en la evaluación, diagnóstico y tratamiento de los quistes hepáticos gigantes.
Conclusión: En todos los artículos revisados, el destechamiento y la fenestración laparoscópicos son actualmente los métodos de tratamiento. Podemos afirmar que la laparoscopía debe ser el primer recurso de tratamiento por ofrecer seguridad y bajos índices de recidiva, además de excelente control del dolor postoperatorio y baja morbimortalidad. La elección de otro método de tratamiento dependerá de cada caso.
