Relación de la administración de nutrición parenteral total y la terapia de reemplazo renal en la unidad de terapia intensiva

RESUMEN
Introducción: La nutrición parenteral total (NPT) es importante en los pacientes críticamente enfermos. Sin embargo, las posibles complicaciones de su uso han sido poco estudiadas. Analizamos la relación entre la administración de NPT y el recibir terapia de reemplazo renal (TRR) en la unidad de terapia intensiva (UTI).
Material y métodos: Estudio multicéntrico retrospectivo, en el cual se analizó la base de datos administrativa de cuatro UTI. Las diferencias entre los grupos que recibieron y no TRR y NPT fueron determinadas. Se realizó un análisis de regresión logística para TRR ajustando a varios factores, incluyendo la propensión de cada individuo para recibir NPT.
Resultados: En total 3,491 sujetos fueron incluidos en el análisis. La TRR fue prescrita en 250 (7.1%) y la NPT en 367 (10.5%). Después del ajuste para varios factores, el uso de NPT se relacionó independientemente a la TRR (RM 2.17, IC 95% [1.52-3.11]; p ‹ 0.001). El área bajo la curva ROC del modelo de regresión logística final fue de 0.74.
Conclusiones: En este estudio multicéntrico retrospectivo, la NPT se relacionó de manera independiente a recibir TRR durante la estancia en la UTI.
