Cuál es el verdadero papel de la cirugía en la infertilidad asociada con endometriosis

RESUMEN
Antecedentes: desde la primera publicación acerca de la endometriosis
hace más de 150 años por von Rokitansky se generó una gran
incertidumbre en relación con los diversos aspectos clínicos, moleculares y sociales de la enfermedad, situación que la convirtió en uno de
los padecimientos más desafiantes para los ginecólogos. Los principales
problemas han sido: el dolor pélvico, la dismenorrea y la infertilidad,
sobre todo por su difícil diagnóstico, además de que las estrategias de
tratamiento son hasta cierto punto limitadas. Se ha publicado una gran
cantidad de artículos relacionados con las dificultades de la mujer con
deseo reproductivo y sus opciones de tratamiento.
Objetivo: revisar la evidencia científica actual y lograr el convencimiento de los médicos de primer contacto que tratan pacientes con
infertilidad asociada con endometriosis del abordaje menos invasivo,
y que la cirugía es, incluso, potencialmente perjudicial en muchos de
los casos.
