Anudamiento de catéter de Swan-Ganz en la rama derecha de la arteria pulmonar

RESUMEN
El anudamiento del catéter de Swan-Ganz es una complicación poco
frecuente. Éste es el segundo caso reportado en nuestro país en un
lapso de 20 años. Los factores de riesgo predisponentes son:
colocación a ciegas, uso de catéteres delgados, calentamiento del
catéter, no inflado del globo, múltiples intentos de colocación con
introducción de gran parte del catéter y dilatación de cavidades
cardíacas derechas, los cuales condicionan el desarrollo de asas
y formación de nudos. La localización más frecuente es en aurícula
y ventrículo derechos, aunque también se han descrito en vena
subclavia, tronco y ramas de la arteria pulmonar. Para el
diagnóstico se utilizan: radiografía simple de tórax, fluoroscopia
y ecocardiografía. El abordaje terapéutico dependerá de las
características del nudo y puede ser mediante técnicas no
quirúrgicas y quirúrgicas.
