Prevalencia de enfermedad periodontal grave (EPG) y su asociación con enfermedades respiratorias en pacientes adultos hospitalizados en un centro de tercer nivel

RESUMEN
Antecedentes. La enfermedad periodontal grave (EPG) es un proceso inflamatorio crónico gingival que se asocia a
enfermedades sistémicas.
Objetivo. Determinar la prevalencia de EPG y su asociación con enfermedades respiratorias
entre pacientes hospitalizados en el Instituto Nacional de Enfermedades Respiratorias «Ismael Cosío Villegas» (INER), 2011.
Material y métodos. Se realizó un estudio transversal. La EPG fue diagnosticada por el Servicio de Estomatología. Se usó
la Clasificación Internacional de las Enfermedades en su décima edición (CIE-10). Se ajustó un modelo logístico multinomial.
Resultados. Se incluyeron 3,059 pacientes; 772 (25%) presentaron EPG. Después de controlar por edad, sexo, días de
estancia hospitalaria, defunciones y nivel socioeconómico, las enfermedades respiratorias infecciosas que se asociaron
significativamente con EPG fueron: infección por VIH/SIDA (RR = 10.6, IC 95%: 9.1-23.3, p ‹ 0.0001); neumonías (RR = 2.6,
IC 95%: 2.2-5.7, p ‹ 0.0001); tuberculosis pulmonar y secuelas (RR = 2.1, IC 95%: 1.6-4.9, p ‹ 0.0001), y abscesos pulmonares
(RR = 2.6, IC 95%: 1.6-7.8, p = 0.002). Los tumores malignos y las enfermedades pleurales se asociaron significativamente
a EPG.
Conclusiones. Se observaron altas prevalencias de EPG que variaron por enfermedad respiratoria. La EPG
se asoció tanto a enfermedades respiratorias infecciosas como no infecciosas. Es importante hacer un estudio de intervención
en salud oral.
