Lesión hepática por isquemia/reperfusión: mecanismos, vías de activación y perspectivas futuras

RESUMEN
Cada 13 minutos se agrega un nuevo nombre a la lista de receptores de órganos y cada día fallecen 17 personas de la lista de espera. La diferencia entre el número de donantes y pacientes en espera de un hígado sigue siendo la principal limitación del trasplante hepático; esto ha llevado a ampliar los criterios de los posibles donantes y a tomar órganos marginales. La disfunción o falla hepática continúa siendo un problema clínico significativo posterior al trasplante hepático, resecciones de tejido hepático y choque hemorrágico. A pesar de los grandes avances que se han logrado para intentar esclarecer los mecanismos de la lesión por isquemia/reperfusión (IRI por sus siglas en inglés ischemia-reperfusion injury), sigue siendo necesario comprender más a fondo la fisiopatología de este proceso dañino para las células hepáticas. La lesión por isquemia-reperfusión es un fenómeno mediante el cual el daño celular en órganos con hipoxia, ya sea caliente o fría, se acentúa posterior a la restauración del flujo de oxígeno a dicho órgano. El preacondicionamiento hepático es una técnica prometedora en la prevención de la IRI.
