Manejo anestésico en niños sometidos al cierre de la comunicación interauricular con el dispositivo Amplatzer®

RESUMEN
Objetivo. Informar la experiencia en el manejo anestésico de niños sometidos al cierre de la comunicación interauricular (CIA) por cateterismo cardiaco, usando el dispositivo Amplatzer.
Pacientes y métodos. Se reportan las observaciones hechas en cinco niños, entre 7 y 15 años, con CIA. Se administró inducción con sevoflurano por mascarilla facial y administración intravenosa de midazolam/nalbufina y durante el procedimiento se administró sevoflurano como mantenimiento anestésico por la mascarilla laríngea hasta el cierre del defecto.
Resultados. Sólo hubo un caso que presentó bradicardia (62/min) al colocar el dispositivo de Amplatzer en la CIA, la cual mejoró con aumentar la concentración de O2 al 100%. La emersión anestésica en todos fue entre 5 a 10 minutos después de finalizar la administración de sevoflurano. En todos se retiró el transductor transesofágico y la mascarilla laríngea sin complicaciones.
Conclusiones. A pesar de ser una experiencia limitada a cinco casos, el uso de la mascarilla laríngea en niños parece ser una buena alternativa a la intubación endotraqueal para el cierre de la CIA con el dispositivo Amplatzer por cateterismo.
