Estándar de oro en el manejo del pie diabético: yeso de contacto total

RESUMEN
Las complicaciones del pie representan uno de los principales problemas de salud de los pacientes con diabetes
mellitus, y la úlcera es una de las lesiones más habituales. Dichas ulceraciones se originan por pequeños traumatismos
repetidos sobre un pie neuropático. El manejo inadecuado del pie diabético (PD) puede conducir a una de las
complicaciones más graves del paciente diabético: la amputación. La clave para el tratamiento de las úlceras plantares
consiste en el control de las comorbilidades (niveles de glucosa y enfermedad vascular), el desbridamiento, el manejo
del exudado con los diferentes apósitos existentes, el manejo de la infección y la descarga del pie afectado principalmente.
Un error común en el tratamiento básico es el método de descarga, que tiene como resultado el retraso en la
cicatrización y la realización de la amputación. Para este fin proponemos el yeso de contacto total (YCT), considerado
el estándar de oro en la descarga del PD.
El objetivo de la presente revisión es presentar la evidencia existente en la literatura médica sobre la efectividad del
uso del YCT para la cicatrización de las úlceras del PD y, por tanto, para prevenir amputaciones.
